We provide comprehensive healthcare which is easily accessible, affordable, and of the highest quality to low income, uninsured, and medically underserved children from infancy to age 19. We also provide health care for adult patients at our Washington Neighborhood clinic.

Additional Services
Patient Navigator
Will assist you in finding available services in your community; provide navigation support for health care services and insurance enrollment, as well as serve as liaison between our patients and outside agencies (i.e., Santa Clara Valley Medical Center, WIC, and Head Start).
Health Education
We assist children, teenagers and their families in learning about a variety of health related topics. Topics include promoting healthy behaviors, growth and development, and preventing disease. Overall, health education teaches our children how to take care of their bodies so they can live a healthy, and hopefully long, life.
Who Staff Our Clinics
Under the direction of our medical director, the clinics are staffed by licensed physicians, nurse practitioners, dentists and bilingual English/Spanish medical assistants.
Who we serve
We are committed to serving low income, uninsured, and medically underserved Santa Clara County children from infancy to age 19 in 4 clinics and for children and adults at our Washington Neighborhood clinic.
In 2014, The National Committee for Quality Assurance recognized our five clinics as Patient Centered Medical Homes.
