VNA Home Health (VNA) is a certified home health care agency providing skilled nursing care, rehabilitative services (physical, occupational, speech pathology therapies), and medical social work to individuals in the comfort of their homes. These individuals have conditions that are acute or chronic and require the skill and compassion of our dedicated clinicians. VNA Home Health provides care to residents in 11 Capital Region counties: Albany, Columbia, Fulton, Greene, Montgomery, Rensselaer, Saratoga, Schenectady, Schoharie, Warren and Washington.NA Home Health has offices in Albany NY and Saratoga Springs. Visiting Nurses Home Care (VNHC) is a licensed home care service agency which provides supportive services including certified home health and personal care, homemakers, companions, licensed practical nursing, and registered professional nursing. In addition, VNHC provides care management services for individuals who are enrolled in specific Managed Long Term Care (MLTC) programs. VNHC provides services in 18 counties including: Albany, Clinton, Columbia, Delaware, Dutchess, Essex, Franklin, Fulton, Greene, Hamilton, Montgomery, Otsego, Rensselaer, Saratoga,, Schenectady, Schoharie, Warren and Washington. The main office is located in Menands, with satellite offices in Glens Falls, Saratoga, Johnstown, Catskill and Cobleskill. VNA Foundation was established to aid and assist VNA Home Health. Foundation funds are utilized to provide home health care services to the medically indigent patients served by VNA Home Health. Funding sources for the foundation include annual appeals; donations from patients and/or their family members in memory of a loved one or in honor of their nurse or therapist; employee donations, and grant solicitations.
