WELL CARE APPOINTMENTS
To schedule an appointment, call 704-752-2000 during office hours. This includes well care and physicals.
Please be prepared with a desired day and time for the appointment along with your name, telephone number, child’s name, and date of birth.

SICK VISIT & WALK-IN APPOINTMENTS
Please notify us earlier that day if you expect to come in without an appointment.
Sick patients will be seen as quick as possible.
Walk-ins are reserved for current patients only and may experience an extended wait as we tend to scheduled and sick patients first.

NEW PATIENT APPOINTMENTS

There you will see a Forms tab. Please print and complete the 3 New Patient Forms.
Arrive 15 minutes before your scheduled appointment with the completed forms to allow for processing time.
You can also fax the new patient forms 24 hours in advance. 704-752-1212
